Assignment Scenario

After graduating from nursing school and passing your boards, you accepted a position as an RN in the emergency center at Rasmussen General Hospital (RGH). In the weeks ahead, your emergency center plans to implement a new initiative where clients diagnosed with low-risk deep vein thrombosis (DVT) will be sent home on 3 months of oral anticoagulation therapy with rivaroxaban.

Historically, clients diagnosed with DVTs in the emergency center were admitted to the hospital’s observation unit. However, when surveyed about their experiences, many clients reported that this was costly, time-consuming, and inconvenient. Under RGH’s new program, emergency center nurses will be responsible for educating clients about their new medication and administering the first dose to the client before discharge.

As part of this new initiative, all emergency center nurses at RGH must be trained to properly educate these clients on all aspects of their new medication regimen. Because you have demonstrated a keen interest in pharmacology, your manager has asked you to develop the educational tools that will be used to train the nurses at RGH.

It’s worth noting that the standard dosing for all low-risk DVTs is as follows:

  • 15 mg BID, 12 hours apart, for 21 days
  • After 21 days, use 20 mg once per day
  • After 90 days, the client will follow up in the thrombosis clinic to evaluate if they meet criteria for discontinuing the medication
Assignment Instructions

Your manager explained that all of the emergency center nurses at RGH will be educated in small groups through a short, in-service training session. You have been asked to develop a professional-looking PowerPoint presentation that the trainers will use to guide the in-service sessions. Your PowerPoint will need to:

  1. Be professional-looking, have a title slide, and 10-15 content slides.
  2. Use the Speaker Notes feature in PowerPoint to reduce the amount of text that appears on the content slides.
  3. The Speaker Notes for each slide will contain the text that will guide what the trainer will say while presenting the slides during the in-service.
  4. The content slides themselves should be clear and easy to understand; they serve as an outline so the audience can follow along with what is being presented.
  5. Be written using proper spelling/grammar and at a level that would be appropriate for the audience.
  6. Remember the purpose of the training is to help nurses learn to educate their clients about the use of a medication. With that in mind, it may be helpful to provide examples of how to explain the information to a client in an easy to understand way without complicated, technical terms.
  7. Include APA formatted in-text citations for at least 3 scholarly references and present the sources in APA format on a References slide.
  8. Contain content addressing the items below.
  9. Pharmacotherapeutics
  10. State the drug name and its therapeutic category.
  11. Pharmacodynamics – how does this drug work to achieve its intended response?
  12. Pharmacokinetics – how is this drug absorbed, distributed, metabolized, and excreted?
  13. Discuss dosage, scheduling, route, and length of therapy.
  14. Describe what clients can expect regarding their therapeutic response and when it should develop.
  15. Introduce nondrug measures that clients can do to enhance their therapeutic response (e.g., specific diet to follow, exercise requirements, substances to avoid, special precautions to take, etc.).
  16. Side Effects, Adverse Effects, and Interactions
  17. Explain the symptoms of common side effects, and what clients can do to minimize any discomfort.
  18. Explain the symptoms of any major adverse effects, and what clients should do if they experience them.
  19. List the major adverse drug-drug interactions and drug-food interactions that clients need to be made aware of.
  20. Follow-up Care
  21. Provide information regarding when clients should call their doctor for advice (e.g., questions, non-urgent symptoms, unpleasant side effects, etc.).
  22. Emphasize any symptoms for which clients should seek immediate care (e.g., when to go to the emergency center or call 911).

Note: When developing a professional-looking presentation, it’s important to be mindful of how you layout your content. The slides should present key points, arranged in a logical manner, without extraneous information contributing to a cluttered look. Using the Speaker Notes feature in PowerPoint is a great way to include relevant details without overloading the slides.

Does a flood of immigrants flood the labor market and drive down wages in the U.S?  Acts 17:26 says, “And He has made from one blood every nation of men to dwell on all the face of the earth and has determined their pre-appointed times and the boundaries of their dwellings (New King James Version, 1982).”  In other words, God has established boundaries where they are to reside.  Thus, there is some biblical reason for people to remain in the nations of their origin.  When God spoke to the children of Israel and referred to people as “strangers,” they were not specifically speaking of immigrants, but rather people who did not share their Jewish heritage but were already among them.  In a journal article in The Review of Economic Studies, it says, “As for the effects on native wages, we find a pattern of effects whereby immigration depresses wages (Dustmann et al., 2012).”  There are several patterns throughout history that confirm this statement, and one example takes place in the early 20th century when thousands of Irish immigrants came to the U.S. looking for work.  An article from the Library of Congress says, “As Irish immigrants moved inland from eastern cities, they found themselves in heated competition for jobs. The audio recording, Immigrant Laborers in the Early 20th Century Links to an external site., describes how West Virginia coal operators fired union laborers and gave the jobs to Irish,”  the article further says,  “some businesses took advantage of Irish immigrants’ willingness to work at unskilled jobs for low pay (Joining workforce).”  In other words, the massive influx of immigration did cause there to be an abundance in the supply of unskilled and menial labor and since the demand did not increase, they ended up taking the jobs of many citizens who were already here and were also unskilled since they were willing to work for far less, so the people here already had a difficult choice, either agree to the lesser wage or leave it to someone else who wanted it.  Now, some immigration is a very good thing, however, when it remains unchecked as it has in the past, or if massive illegal immigration occurs, it can lead to massive disruptions in the labor market since it will have been flooded.  Also, when the labor market was flooded and wages went down for it, it forced the parents to need to send their children to work in factories since they were not making enough to make ends meet anymore.  The conditions were horrible, and children ended up not being able to get a decent education because of it, thus resulting in them remaining unskilled workers for the rest of their lives (Horrell & Humphries, 1995).  We should be looking for the best and the brightest to immigrate here.

The U.S. was founded with the idea that all people are created equal and that it could be a haven for people from all over the world looking to forge a better life. As time has progressed, the topic of immigrants coming to the U.S. in recent times has become a heated political topic, with fifty-two percent of registered voters, according to PEW research, saying that this issue will impact their vote (Pew Research Center 2020). Regarding immigration in the U.S., we will primarily focus on the southern border because, in the Fiscal year of 2022, CBP stopped more than 2,766,582 migrants trying to illegally cross the border into the U.S. (Ainsley, 2022). Approximately 11 million Mexican immigrants have been living illegally in the U.S. since 2015; it is likely much higher now (Lessem, 2018). After reading an article from The University of Akron, I believe that regarding the charge that immigrants flooding the labor market and driving down wages in the U.S. is that there are no significant negative effects on wages in the U.S. . The reasoning behind this decision is that when there is a large flood of immigrants into the labor market, they are most likely low-skilled workers who are entering the U.S. due to a shortage of workers in a particular market (Desai, 2020). Also another reason they are immigrating is due to the fact they are being paid more in the U.S. than in their country of origin. For instance, Mexico has a minimum wage per day of $8.88; in border areas, it is as high as $13.33 per day, which ends up being significantly lower than the U.S. minimum wage of $7.25 per hour, with most workers working 8-hour shifts a day resulting in higher pay (Mexperience, 2022). However, this problem of large amounts of illegal immigrants does have a chance for several long-term negative effects, such as immigrants sending their money home to their families and not spending it here in the U.S. and social programs and welfare. Some positives can offset these negatives, such as the economic benefit generated from immigrants’ jobs. If they stay longer in the U.S., they might even pay taxes paying back into social and welfare programs.
